Diana Leszczewicz, FNP

Diana joined our team in 2020 and was recently promoted to Clinical Director. Patients appreciate Diana’s friendliness, knowledge, and experience, and respect that she listens to their concerns. She is a dedicated patient advocate and goes the extra mile to ensure patient needs are met.

Shelby Groves Sharp, FNP

Shelby is one of our valued Family Nurse Practitioners who was born and raised in the community she now proudly serves. With four years of clinical experience, she enjoys building meaningful relationships with her patients and is passionate about providing thoughtful, personalized care close to home.

Sheena Blake, FNP, PMHNP

Sheena began her career as a Family Nurse Practitioner, later continuing her education into psychiatric mental health. Sheena continues to provide primary care as our Family Nurse Practitioner one day a week. With six years of experience, she brings a well-rounded, compassionate approach to caring for her patients. 

Lorissa Dias, FNP

Lorissa has been practicing as a Family Nurse Practitioner for ten years and specializes in Pediatric Care. She is passionate about supporting children’s health and partnering with families to help kids grow, thrive, and feel their best.

Sheena Blake, PMHNP

While serving as our Behavioral Health Director and Psychiatric Mental Health Nurse Practitioner, Sheena specializes in medication management and mental health services. She has lived in our community for sixteen years and is passionate about supporting patients’ mental wellness with compassion and personalized care. 

Mark Monroe, LCSW

Mark has been a Licensed Clinical Social Worker for seven years. He specializes in therapeutic counseling and offers a wide range of tailored services to meet the unique needs of each patient. He works collaboratively with patients to set goals, track progress, and provide education to enhance overall mental wellness. 

Brittney Levsey, DMD

Dr. Brittney Levsey has been practicing as a Dentist for twenty-two years and has been providing excellent dental services at our clinic since 2018. Her extensive knowledge in the dental field allows her to provide expert dental care to her patients and build lasting relationships with the patients she serves. 

Cedar Baik, DMD

Dr. Cedar Baik has been practicing as a Dentist for two years. He is committed to providing high-quality, patient-centered dental care and takes pride in helping patients feel comfortable and confident in their smiles. 

Marcella Maloney, RDH

Marcy has been a Registered Dental Hygienist for ten years. She specializes in advanced periodontal cleaning, sealant placements, and routine cleaning. Although Marcy sees all patient ages, her prior experience has assisted her in providing gentle, expert care for children. 

Teresa Cardwell, RDH

Teresa has been a Registered Dental Hygienist for thirty-five years, specializing in every area of dental hygiene. Her years of service are applied to every patient with compassion and well-rounded care.
